Replacing the adapter and blade seemed to be a good idea, so I went to our nearby Sears Essentials to get new ones, but they were out. If overtorqued, the bolt is under more stress than it was designed for. Torque to about 35 ft lbs, and the reverse threads will tighten it if more if needed. If the star hole is widened out in the blade or if the blade is bent or out of balance, it can cause it to tear up the adapters. It is possible it wasn't quite tight enough the first time you tightened it and it cracked or damaged the key. If it isn't, take the adapter back to where you bought it and get a new one.
